Behavioral Health Care, Rooted in Our Community

Harbor Reach Care provides behavioral health and community based services to individuals and families in East Harlem and surrounding New York City communities.

Our team works with each person to understand their behavioral health needs, connect them with appropriate care and provide ongoing support throughout their journey.

Care That Meets People Where They Are

Harbor Reach Care is a community based behavioral health organization serving East Harlem and surrounding communities.

We believe quality behavioral healthcare should be accessible regardless of a person's income, background or circumstances.

Our approach brings together behavioral healthcare, peer support, case management and community resources to help people receive the support they need and stay connected to care.

Support Beyond the Clinic

Health and wellbeing are influenced by much more than what happens during a medical appointment. Our team works to understand the challenges affecting each individual and help connect people with available community resources and support.

Depending on individual needs, this may include assistance navigating:

  • Healthcare services
  • Benefits and coverage
  • Housing resources
  • Food and nutrition resources
  • Transportation resources
  • Community programs
  • Social services
  • Recovery resources
  • Other community based support

Our Mission

Our mission is to improve behavioral health and overall health outcomes for underserved communities by making compassionate, high quality care more accessible and helping individuals overcome the barriers that can prevent them from getting and staying connected to the support they need.

We believe everyone deserves the opportunity to achieve better health and wellbeing, regardless of income, insurance status, background or life circumstances.

Our Vision

We envision healthier, stronger communities where every person has equitable access to behavioral healthcare and the support needed to live a healthy and fulfilling life.

Our vision is a healthcare system where people do not fall through the cracks because of financial hardship, housing instability, transportation challenges or difficulty navigating a complicated system of care.

Whole Person, Community Based Care

Person Centered

Every person's circumstances are different. We work to understand individual needs, goals and barriers to care.

Accessible

We are committed to reducing barriers that make it difficult for underserved communities to access behavioral healthcare.

Connected

Behavioral health does not exist in isolation. We help connect individuals with healthcare, social services and community resources when additional support is needed.

Continuous

Getting someone through the door is only the beginning. We believe in helping people remain connected to appropriate care and support throughout their journey.
